  1. This information packet linked below contains all of the materials necessary to participate in the RIASMD 2024/2025 Scholarship program.
  2. The information required by this scholarship application will need to be completed by both the applicant and his/her school principal or counselor.  As the instructions indicate, Parts I, II and III. Will need to be completed by the school principal or counselor with the applicant completing parts IV, V and VI.
  3. Completed application forms should then be mailed to the Scholarship Program Director at the address indicated below.
